Practice request

For practices and employers. Submit your company name and phone for a provider's listing. Each request is reviewed. Once approved, you can pay $10.99/month for that listing to manage its public details. A practice cannot claim or own a provider's profile, cannot change the provider name or NPI, and the listing shows as Verified once the practice's subscription is active. The provider can also claim and edit the same listing; patients see the most recent save.

The $10.99 individual PMHNP.com membership, including the member card, is for the provider after their own claim is approved and their subscription is active. A practice pays separately to manage the listing.
